In the 2016 Community Health Needs Assessment, Clinical Preventative Services, Obesity/Nutrition/Physical Activity, and Social Determinants of Health were three of the top six health topics prioritized by respondents. In response to these rankings, and based on SC having the 7th highest prevalence of diabetes among adults in the nation, TUW’s Guiding Team for Health made the recommendation that the organization focus attention on the reduction of pre-diabetes and diabetes in the Tri-County area.
Launched in January 2017, the Tri-County Diabetes Coalition is a partnership of healthcare providers, community organizations, faith-based groups, local government, universities and community members committed to increasing education, raising awareness about, and reducing rates of Pre-diabetes and Type 2 Diabetes in Berkeley, Charleston and Dorchester Counties.
Focus: Preventing new incidences of diabetes and pre-diabetes in the Tri-County region through the promotion of Diabetes Prevention Program (DPP), a CDC-recognized lifestyle change program designed to prevent Type 2 Diabetes
Target Participants: Staff from multi-sector organizations with a background or interest in diabetes and pre-diabetes and nutrition/obesity/physical activity
Commitment: 2-4 hours per month (both time within quarterly meetings and independent work efforts)
