Healthy Tri-County Workgroups

Healthy Tri-County (HTC) engages individuals and organizations from various sectors that are interested in improving health outcomes in the region. Members of HTC actively collaborate with a range of partners to develop and implement actions and strategies that will positively impact health outcomes in Berkeley, Charleston, and Dorchester Counties in South Carolina.

Collaboration: Network and establish relationships with the more than 70 organizations actively supporting the efforts of HTC
Action: Support the development of strategies to improve regional health outcomes, and engage in various educational, professional development, and networking events
Impact: Become a part of the solution by committing your personal and organizational time, talent and resources to achieving measurable and sustainable change in the Tri-County

Executive Committee

FOCUS: Provide strategic direction to HTC and approve the development and monitoring of the Tri-County Health Improvement Plan
TARGET PARTICIPANTS: CEO or other designated C-suite or VP level staff members with decision-making authority
COMMITMENT: 2-4 hours three times per year (both time within meetings and independent work efforts)

Tri-County Health Improvement Plan (TCHIP) Workgroups

FOCUS: Implementation of the Tri-County Health Improvement Plan (2018-2023). Subcommittee and Workgroup teams based on the Framework below implement the activities and strategies of the plan as well as to continue to identify common measures, language, and goals for inclusion in the plan
TARGET PARTICIPANTS: Staff from multi-sector organizations able to provide input to strategic directions and help craft regional health improvement plan
COMMITMENT: 4-6 hours per month (both time within monthly meetings and independent work efforts)

Health Data

FOCUS: (1) Continue the work on standardized Community Health Needs Assessment (CHNA) implemented tri-annually by multiple organizations in alignment with Roper St. Francis for the Tri-County region and (2) Maintenance and improvement of the local health resources hub created by the workgroup 
TARGET PARTICIPANTS: Staff from multi-sector organizations with a background or interest in data collection, visualization and dissemination
COMMITTMENT: 4-6 hours per month (both time within monthly meetings and independent work efforts)
